Applying for Services

To register with ODS, you will need to have an intake interview with Melanie Bukovsky, the Advocate for Services to Students with Disabilities. Call the Middletown office at (845) 341-4077 to set up an appointment. You may also call Ms. Jennifer Clayton, our Newburgh Advocate for Services to Students with Disabilities/Learning Communities Advisor at (845) 341-4034.

At the time of the intake interview, students must present appropriate medical or psychological documentation for the stated disability. Documentation must be recent, relevant, and must establish a clear link between the disability and the need for specific accommodations, like calculator use, extended time for tests, etc. Typically, documentation generated within the past three(3) years is considered current. For students with a suspected learning disability, referrals can be made for off-campus diagnostic testing and evaluation at the student's cost.
