The insurance is a secondary policy that will pay medical costs over and above a $25 deductible, up to $5,000 for out-of-pocket costs not covered or reimbursed by your primary insurer. GIRLS� SOCCER LEAGUES LEAGUE DIRECTOR Andrea Conklin Orange will hold its annual Girls� Soccer League at Orange County Community College. There will be two divisions, one for High School students and one for girls who are graduating this spring and older. REGISTRATION DEADLINE FOR BOTH DIVISIONS IS MONDAY, JUNE 26, 2006. Schools wishing to play together as a team may enter one application and list all players on that team. Addresses and telephone numbers along with social security numbers and dates of birth are also needed on this form. Individuals who wish to play with other friends may request to do so by listing those players on their own form, although each player must fill out their own form.
