I have REGISTERED for classes or I will notify the Financial aid Office when I have registered. I have met the IMMUNIZATION REQUIREMENT (If uncertain, contact Campus Health Service Office). I have completed the 2007-2008 FREE APPLICATION FOR FEDERAL STUDENT AID and/or the 2007-2008 RENEWAL FREE APPLICATION FOR FEDERAL STUDENT AID and submitted it. I must COMPLETE the separate TUITION ASSISTANCE PROGRAM APPLICATION (for full-time students only - 12+ credits) online or the one, which will be mailed to me as a result of filing for Federal Aid. I should contact the Office of Financial Aid to complete the separate AID TO PART- TIME STUDY APPLICATION (APTS), if I am a NYS resident and plan to enroll part- time.
