	2007-2008 FINANCIAL AID APPLICANT CHECK LIST

     	I have made or will make APPLICATION FOR ADMISSION To SUNY ORANGE and            	have taken the COLLEGE ASSESSMENT TEST.

           I have REGISTERED for classes or I will notify the Financial aid Office when I have                          registered.

     	I have met the IMMUNIZATION REQUIREMENT (If uncertain, contact Campus 		Health Service Office).

     	I have completed the 2007-2008 FREE APPLICATION FOR FEDERAL STUDENT AID                 and/or the 2007-2008 RENEWAL FREE APPLICATION FOR FEDERAL 		STUDENT AID and submitted it.

     	 I must COMPLETE the separate TUITION ASSISTANCE PROGRAM APPLICATION 
(for full-time students only - 12+ credits) online or the one, which will be mailed to me as a         result of filing for Federal Aid. 

     	I should contact the Office of Financial Aid to complete the separate AID TO PART-		TIME STUDY APPLICATION (APTS), if I am a NYS resident and plan to enroll part-  
	time.


 	
	REQUIREMENT FOR STUDENTS SELECTED FOR VERIFICATION

     	I understand that if selected for verification, I must provide my student & parents 		                   and/or student and spouse) income documentation that is applicable:

      	Signed photocopy of Students 2006 Federal Tax Return (1040, 1040A, 1040EZ) and W2�s
      	Signed photocopy of Parents 2006 Federal Tax Return (1040, 1040A, 1040EZ) and W2�s
      	Verification of ALL NON-TAXABLE INCOME (Calendar Year 2006)
            - Social Security Benefits
            - Direct Social Service Benefits (Budget Sheet Not Acceptable)
            - Child Support Statement (2006, for all children)
            - Any other non-taxable calendar year benefits received by family
              (i.e., Workman=s Comp., Disability, etc.)

	IF REQUIRED TO SUBMIT INCOME INFORMATION, 
	PLEASE MAKE SURE STUDENTS NAME AND 
	SIGNATURE APPEARS ON ALL
	INCOME DOCUMENTATION
