Instructor Script

This evaluation is anonymous and administered under the direction of the Vice President of Academic Affairs, not the course instructor.  

The results of the Student Feedback Forms will be provided to each instructor only after all final grades are distributed to students.   

Therefore, your answers and comments will not affect your final exams or course grades in any manner.  The information obtained through this evaluation is intended to be used to make future improvements in this course.

Please complete the Student Feedback Form individually and without talking.  

When you have completed the evaluation, please hand it to:  (Student Administrator).
