Below you will find several important links:
- When the application is open you will find the link to the Health Professions Application
- The Resource Page for each Health Professions program which details the Admissions and Department Specific requirements.
- Information about the mandatory pre-admission seminar.
- Select programs require Observation Hours and you will find the appropriate forms below that must be completed, signed, and submitted with your application.
All Nursing applicants must submit ATI TEAS exam results from within 2 years prior to the application deadline. Applicants will find links to the TEAS study guides, and sign-up page to take the test.
Dental Hygiene (Middletown)

Students must submit both of the forms below and must complete a minimum of 4 hours
in-person observation and complete the virtual observation & activity. Our forms can

Students must attend a pre-admission seminar within 3 years prior to the application
deadline.
Medical Laboratory Technician (Middletown)

Nursing LPN to RN (Fall Newburgh; Spring Middletown)

4. Nursing Content Master Series Test
Students who are outside the articulation agreement must complete the Nursing Content
Mastery Series Test and score Level 1 or higher. Students can access study guide and
materials here:
Occupational Therapy Assistant (Middletown)

the OTA program.
3. Fall 2024 pre-admission seminar coming soon!
Students must attend an in-person pre-admission seminar within 3 years prior to the
application deadline.
4. Potential Interview
Students who meet the academic and department specific eligibility criteria may be
invited for a group interview with the OTA department. At this meeting students are
asked to do a writing activity and an essay will be requested. Interviewees are selected by the department.
Physical Therapist Assistant (Middletown)

Must be printed and submitted to Student Services or emailed to apply@sunyorange.edu
4. Fall 2023 observation form
A pre-entrance requirement is the completion of 48 hours minimum of clinical observation within 3 years prior to the application deadline.
An essay is also required with the observation form.
5. Potential Interview
Students who meet the academic and department specific eligibility criteria may be
invited for an interview with the PTA department. Interviewees are selected by the
department.
Radiologic Technology (Middletown)

application deadline.
