Information for Students Entering a Health Professions Program in Fall 2021
Physical Packets for incoming health professions students were emailed in late April. Check your SUNY Orange email account and if not received check your spam. If you do not receive a packet please email email@example.com.

For Students Entering the Second Year of a Health Professions Program
Physical Packets for returning health professions students were emailed in May. Check your SUNY Orange email account and if not received check your inbox and spam. If you do not receive a packet please email firstname.lastname@example.org.

Submitting Your Physical
All physicals must be MAILED to the SUNY Orange Wellness Center, 115 South Street, Middletown, NY 10940.
MAKE A COPY of all documents before mailing, and make sure your return address and sufficient postage are on the envelope. The Wellness Center and College are not responsible for physicals lost or damaged in transit. You do not want to be the student whose documents were lost or damaged and there are no copies.
The mail is picked up twice a week. When your physical is received, a nurse will send an email to your SUNY Orange email account confirming receipt. Check your spam as well as your inbox. If you do not receive this email more than 10 business days after mailing your documents, please email us at email@example.com. Keep in mind that the postal service has been slow delivering mail due to COVID-19.
Receiving Your Clinical Clearance
Once your documents have been reviewed and deemed compliant with New York State requirements for healthcare personnel, the nurse will issue you an official clearance form which will be emailed to your SUNY Orange email account. This process may take anywhere from one to two weeks after your documents are received. As our staff is small, please be patient and wait to hear from us. Time spent responding to calls and emails regarding a student's physical status takes time away from processing physicals.
ATTENTION: The issuance of clinical clearances is on hold as we await guidance from SUNY regarding the COVID-19 vaccine. Once this issue is resolved the clearances will be sent to all students who have met the medical requirements for healthcare personnel. Please complete your physical forms and send them to the Wellness Center as per the previous guidelines and submission dates. All deadlines for receipt of physicals remain in effect.
If your documents are incomplete or do not meet the New York State requirements, you will be contacted via email. Incomplete documents may be returned to you via mail, delaying processing and your clinical clearance.
